How Does Scarring Happen?
Scarring is a natural part of the healing process after an injury. Its appearance depends on multiple factors. The depth and size of the wound or cut and the location of the injury matter. How the injury was created whether it was a cut, thermal or chemical injury. Age, genetics, gender and ethnicity all play a role. The most important factor is what you do next after the injury!

The real name of a scar is Fibrin Glue. The body will deposit this substance on each side of the injury. If an injury is larger more Fibrin Glue is needed to close the wound thus this may lead to a wider scar. All human beings produce the same chemicals. Therefore, all humans produce Fibrin Glue. The difference between human beings is not the substance we create but rather, how much we create. The goal of any treatment of scars is to produce the most optimal scar if you have to have a scar and if possible hide the appearance of a scar.

What Are The Types of Scars?
Several different types of scars are possible when you heal. The question is which one will yours be? If you are interested in affecting the outcome of a scar, then you must influence the outcome of a scar BEFORE you heal!

Widened Scars: Where the body produces too much fibrin Glue causing the scar to widen Hypertrophic scars: Another subset of humans produces too much fibrin glue where the scar does not widen but the glue mounds to form a “speed bump” appearance or an elevation. These are raised scars but do not go beyond the boundary of the injury.
Keloid scar: These scars are the rarest of all scars. This occurs in a small subset of the population of humans. Because of reasons that remain unknown their body does not stop producing fibrin glue. As a result this scar continues to mound upward until the forces of gravity causes the scar to fall backward toward the body creating a “mushroom” type of appearance. Keloid scars are most common among people with dark skin.
Contracted scars: These scars tighten, shorten, constrict and pull the skin, which can impair your ability to move. Contracted scars may also go deeper, affecting muscles and nerves. Most common these occur from burns either chemical or Thermal.
Acne scars: If you’ve had severe acne, you probably have the scars to prove it. There are many types of acne scars, ranging from deep pits to scars that are angular or wavelike in appearance. Treatment options depend on the types of acne scars you have.

What Are Possible Treatments for Scars?
Scar treatments may include:
Surgical removal or treatment: There are many options to treat deeper scars depending on your particular case. These include skin grafts, excision, dermabrasion, lasers or surgery. In a skin graft, the surgeon uses skin from another area of your body. This is often used with people who’ve had burns. If you’ve got scarring that impairs function, surgery can help address the functional problems. If you’ve recently had surgery that has caused scars, it is best to wait at least one to two years before making a decision about scar treatment. Many scars fade and become less noticeable over time.
Injections: You may get steroid injections to treat scars that stick out, such as keloids or hypertrophic scars. Injections can be painful and have other complications.

Other types of injections, such as “fillers,” may be useful for some types of pitted scarring, although these are not usually permanent solutions.

Do I Need a Breast Lift?

Dr. Michael W. Gray, a world-renowned cosmetic surgeon (West Bloomfield, Michigan) was asked this very question. “Many women are confused as to whether or not they ‘need’ a lift. First of all, No one ever has to get a lift unless they want a lift! Many women think they need a lift and do not need a lift”. Why do women think this? Dr. Gray explains, many women wear push up bra’s to obtain fullness at the top of their breasts when in clothing! Because of this the thought among women that a lift is needed invades the female culture when in actuality a lift would not be needed at all.

In this situation according to Dr. Gray, a breast augmentation would produce upper pole fullness. Fortunately, many women are so relieved to hear this but unfortunately, there are many women who live there lives unhappy and are wrongly afraid to do anything because they fear a breast lift.

A Breast Lift (mastopexy) is composed of two components, as Dr. Gray explains. There is the Horizontal incision and the Vertical incision. Together they form the “anchor scar”. These two components play a major role and each achieves different results. Dr. Gray explains, The Horizontal scar is the one that actually creates the lift. The Vertical scar is the one that moves the nipple/areola complex from its “old” location to the “New” location. To do this many surgeons may cut the nipple and areola off and “graft” the nipple/areola to the new location. Dr. Gray remarks, “That in all my years in surgery, I have never had to do this!” This method is called a “Free Nipple Graft”. Free Nipple Grafting is almost 100% unnecessary when doing a Mastopexy.

Dr. Gray has developed a procedure where the nipple and Areola will remain attached to your breast tissue and breast-feeding will never be altered. The vertical portion of the scar is where the Nipple/Areola was located and the new location you see is where the nipple/areola goes. The lift of the nipple is limited by the length of the blood vessels supplying the nipple/areola. The Horizontal Incision is the portion of the procedure that actually will eliminate the breast from hanging over the crease below. The crease is called the inframammary crease. This horizontal portion of the procedure is what actually lifts the breast. To understand this better, think of the hanging portion of your breast as your shirt hanging over your belt. If the shirt is tucked in, then there is no more hang. To tuck the shirt inward the excess along the horizontal must either be removed or hidden.

When performing a breast lift the portion is hidden and not removed. When the shirt is tucked inward the hang is gone, but is the shirt gone? The shirt is not gone! The shirt is hidden! Wah Lah! No more excess. The portion of the breast and skin that hangs over the Inframammary Crease is not gone but pushed upward as the skin is tightened. Now that you have some basics, Lets understand if a lift is needed.

If a person has a Low nipple but the breast is not hanging over the crease, they may get away with just the vertical scar (the vertical lift portion of a mastopexy). On the other hand, If someone has a breast hanging over the crease and a low or potentially low nipple they would need both portions of the mastopexy (Horizontal and Vertical incisions). If someone has a breast that hangs over the crease and wants a breast implant, but does not wish a Breast Lift (Mastopexy) then more than likely they will place the implant over the muscle. In this situation if the implant is not placed over the muscle (which means behind the breast) they will most likely develop a “Snoopy dog” deformity. The “Snoopy Dog Deformity appears as a dangling breast, or droopy look.

This occurs because the implant is under the muscle verses over the muscle. This droopy look is similar to the appearance of a water droplet getting ready to drop off the side of a glass. This can be corrected either with a lift or by placing the implant over the muscle if a lift is not desired. So, the real question comes down to whether the patient actually wishes a lift, needs the lift or needs the lift to place and implant under the muscle and have the breast actually look good. This is a personal choice with the patient and is the reason why we call Cosmetic Surgery “Elective”. Patients tell me what they want versus me telling the patient what they have to do.

Do I have to be afraid of my Breast Implants?

Then if this is true, how do you explain mold or infections?

There exists many different types and many forms of living organisms in this universe. In fact there are some creatures that are so small that you can barely even see them with the most powerful of all microscopes… the electron microscope!

This telescope can see ( magnify )microscopic particles, hence micro-scope. While the smallest objects that your unaided eye can see are about 0.1 mm long. The light microscope can magnify small objects for you up to 1000 times their original size. The electron microscope can increase magnification another 1000-fold! That is one million magnification. The most powerful electron microscopes allow us to see objects as small as an atom!

In the course of evolution with genetic selection the living organisms will move toward species that will be more fit than prior generations. Evolution ensures the survivability of superior generations of species while the species that are unable to survive disappear. Some of these living organisms have developed abilities to remain dormant until conditions exist in their environment that their survivability would be ensured. Bacteria, yeast (mold) and viruses are capable of dormancy.

All living organisms need to eat. Imagine being eaten by a large organism? An example of this would be being bit by a bear. Of course common sense would tell us that this would hurt. Now, imagine a creature so small that you can not see it but still tries to bite you. Would that hurt as well? An infection is when you are being eaten! A contamination is when the organism is there but not bothering you.

To understand, an infection (whether by a virus, a bacterium or mold) by single celled microscopic organisms is still a process where you are being eaten and that hurts! The most common Infections are by Bacteria. A pimple is an infection where the bacterium is eating your skin. That is why pimples hurt!

The operating room in a true surgical facility has filtration systems where the air that enters the operating room is extensively filtered where no dust, bacteria or mold or viruses are present in the air. This is one of the many processes that prevents infections. In addition to this, Strict sterilization of instruments, sterilization of medication and implants are performed. Preop antibiotics and prepping of the patient’s skin with chemicals that kill pathogenic organisms are used.

Doctors and ancillary staff clean and scrub themselves with these chemicals as well and cover themselves in sterile attire to barricade any organisms that may be present under the sterile garb.

If these processes are followed then how would an infection occur?
One way an infection can occur is when the patient contaminates themselves. Once a patient leaves the care of the physician they enter a world of danger. Patients are unaware that these invisible organisms exist everywhere in the universe. Proper hand sanitizing prior to touching any wound or incision is extremely important.

A second possibility but unlikely, is the failure in the sterilization process. Failure of sterilization with instruments and implants are improbable due to the presence of multiple levels of safeguards such as many indicators that sterilization is complete and successful. These include color indicators on and within the packages that turn color on sterilization. Spore testing in which live attenuated non pathogenic bacterial spores are placed into the sterilizing units and when removed placed in culture medium to see if they grow. No growth indicates death to the organism and that the sterilization process is working. Temperature and pressure indicators also monitor sterilizers with each use.

Personnel and the patient and the patient to themselves is a very low and unlikely cause.

Operating Room: as discussed above true operating facilities have in place extensive filtration systems to eliminate these micro organisms from the air in the operating room. If a physician is in a facility lacking these systems an infection by these microorganisms can occur.

In order for any organism to gain a foothold the organism would have to be introduced either before ( unlikely ) , during ( possible but not as likely ) , or after ( probable ) the procedure. These microorganisms are pandemic to our universe. These organisms are in the air we breathe and the what we touch. They are on the handle to a door, on a coffee cup and on the newspaper you are reading. These organisms can be introduced into the body by breathing, eating, drinking or through a break in your skin at any point in time.

Once these organisms enter your body they can travel in the blood stream to any foreign object in your body ( ie breast implants, heart valves, prosthetic joints, et al). In order to kill these micro-organisms the antibiotic or anti fungal will need to be delivered to the pathogen that is invading your body. To be delivered a blood supply is mandatory for this delivery. Your blood vessels are the roads which allow the blood to deliver these drugs.

Living tissue has a blood supply while implants do not. The infecting invading pathogen will never see an antibiotic nor an Anti fungal once that pathogen reaches and lands upon an implant. Because implants are not alive they do not have a blood supply therefore, the drug that would kill the organism is never delivered. The treatment ultimately becomes removal of the implant. Living tissue that remains has a blood supply so any pathogen left behind would be eliminated if a proper medication is provided. Replacement of the implant can occur safely no sooner than three months after removal.

Eating, breathing and even Brushing and flossing your teeth can cause malignant pathogenic micro organisms access to your body which can enter your blood stream and can infect implants (ie a heart valve or prosthetic knee). Although these occurrences happen in the world of what we see in medicine, these infections are extremely rare. The implant is not food and does not harbor mold ( yeast ) , bacteria nor viruses.
This fear is unwarranted.

EndoBAM is an acronym for Endoscopic Breast Augmentation Mammoplasty and is the endoscopic surgery to enlarge the size of the breast(s). Endoscopic literally means a surgery or procedure performed by the use of an endoscope, which is an instrument for visualizing the interior of the body. Think of an endoscope as a camera which can be inserted into the body. Hanging up the White Coat

White coats have been a fixture in medicine since only the late 19th century. Doctors previously wore black, but switched to white laboratory attire to signal that medicine was science and pure. The coat has also been a symbol of status. The longer the coat the more tenure and power. The long coat easily identified (to ancillary hospital staff) experienced Doctors who are more capable of helping a patient during a life threatening situation.

Currently there are discussions underway to get rid of the white coat. Why? They spread infections.

Although I do not deal with infected hospital patients, I do believe it’s time to rid doctors of these white coats.

Patients that are admitted to hospitals are sick and often harbor resistant “super bugs”.

The reason to leave the white coat in the past is that this coat is not cleaned between patients and serves as a germ magnet! These germs can cause deadly infections from one patient to another. And these bacteria enter the homes of health professionals.

What is brought into the hospital as well as what leaves the hospital is equally important. This can involve change of clothing so that hospital attire is not brought home. These germs then can invade all the clothing and living space in a person’s home which would then be brought back to the hospital to another patient another day.

Are you Smart? Street Smart? or are you Book Smart?
The meaning of “book smart” is that you are DUMB. It represents a derogatory descriptive term to categorize people who have the ability to get an “A” but the inability to apply this knowledge to life. “Street smart” on the other hand, describes a group of individuals that can be taught anything and CAN apply the information learned BUT can not be taught through traditional methods of education. “Smart” describes individuals that can apply the knowledge learned AND get “A’s” on exams. These descriptions hold vital meaning when you look at physicians, standardized testing and the acceptance process to medical school.
Unfortunately, ability can not be judge, nor the intelligence of an individual based on their performance on tests. Getting the right answer does not correlate nor demonstrate skill or intelligence. The recent change in the MCAT’s demonstrates the failure of standardized testing to produce qualified physicians. The continued acceptance of book smart individuals into medical school has produced an array of problems in the health care system. Although one may argue that this is the only method where we can assess an individual, using grades as the measure of acceptance into Medical School has resulted in a massive amount of poor quality doctors. Medical school candidates are not accepted unless they get an “A”. The problem with this is that no one knows that these future Doctors are dumb until they graduate medical school. Only after medical school is the information supposedly learnt applied.
The recent change in the MCAT is an attempt to identify future doctors that can actually think! Time will tell if this will work.
